Mental Health Services


The Eldercare Clinician program provides outreach mental health services to adults age 60 and over in an effort to improve wellbeing, increase or maintain quality of life and maximize independence.


The eldercare clinician provides home-based counseling for individuals experiencing depression, anxiety and adjustment disorders, dementia, schizophrenia and substance use disorders. In addition, eldercare clinicians work closely with medical providers, case management staff and family members in support of older adults experiencing losses, changes in major life areas and issues related to declining health.
